BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2015211264 DESCRIPTION A woman with a 1-year history of diplopia presented to the Ophthalmology Clinic in June 2014. She had a known history of infiltrating ductal carcinoma of the breast with metastasis to the bone and had undergone chemotherapy for this. On the day of her consultation, her visual acuity was 6/9 bilaterally and she was experiencing diplopia in extreme up and down gazes. She was referred for an MRI of the head and orbits, which revealed multiple metastatic deposits to the multiple extraocular muscles bilaterally (figures 1 and 2) as well as within the brain. Although rare, there are accounts of numerous malignancies with metastasis to the orbits, including breast, lung, genitourinary and gastrointestinal tumours. Of these, breast tumours have been reported as the most common malignancy to do so. However, bilateral metastasis to more than two extraocular muscles of this tumour, such as in this case, has been acknowledged as being very unusual. Patients with metastasis to the extraocular muscles usually present with symptoms of reduced vision, diplopia and pain in the region. Additionally, they may experience proptosis and ptosis. The diagnosis can only be confirmed histologically. In this case, the patient underwent a muscle biopsy from the left extraocular muscle, which confirmed the diagnosis of breast cancer metastasis. She was treated with whole brain radiotherapy and a follow-up MRI demonstrated she had had a partial response to this. She currently continues management under oncology.

Zosteriform Skin Metastases from Breast Cancer
Metastatic skin cancer is a rare complication of internal malignancies. Patients who develop cutaneous metastases rarely present with a zosteriform distribution. Herein, we describe a 65-year-old woman with a prior history of breast cancer in whom zosteriform metastases developed along her left T8-9 abdominal dermatomes.
متن کاملDiscrete carcinomatous metastases in the extraocular muscles.
The orbit is a rare site for metastatic carcinoma (Duke-Elder, I952) and, although such orbital deposits may commonly involve muscle secondarily, discrete metastatic tumours in the extraocular muscles are extremely rare, as in muscle generally (Willis, I952, I953). A search of the literature has revealed four previous cases: two from carcinoma of the breast (Wintersteiner, I899; Bedford and Dan...
متن کاملBreast Cancer Metastasis to Pituitary Infandibulum
Metastasis from breast cancer to other parts of the body is very common, but the spread of the tumor to pituitary gland, especially to infandibulum, is a rare presentation. At the time of pituitary metastasis, a majority of the patients have clinical and radiological evidence of the disease. It seems that the posterior area of the gland is the most common site of metastasis, probably due to hig...
متن کاملA Rare Presentation of Colorectal Cancer with Unusual Progressive Intramuscular and Subcutaneous Metastatic Spread
Colorectal carcinoma is one of the most common causes of cancer-related death, worldwide. Recently, due to the introduction of novel imaging and therapeutic techniques, five-year survival of patients has increased. However, distant metastasis is still expected in half of the patients. Colorectal cancer tends to target the abdominal cavity, liver, lungs, and bones as the common sites of metastas...
متن کاملMuscle metastases: comparison of features in different primary tumours
BACKGROUND Muscle metastases (MM) from solid tumours are rare. The aim of this study was to describe radiological features of MM, and to compare their patterns in different malignancies. METHODS A retrospective search in the statistical database of our institution revealed 61 cases of MM. Additionally, a retrospective search in Pubmed database was performed. Together with our cases the presen...
